No proof of income, no ID and no proof of address are required. Generally no forms either: the site will ask how many people are in your household, how many children and how many seniors. A site may offer its own form for internal records, but that is optional. At distributions of federal TEFAP food you will be asked your name, county of residence, household count and whether this is your first TEFAP food this month.
